4. Appointment and Cancellation Policy

Appointments at Forever Rejuvenation Wellness are scheduled in advance. To cancel or reschedule an appointment, you must provide at least 24 hours notice. Cancellations or rescheduling made less than 24 hours before the scheduled appointment time may result in a cancellation fee equal to the full appointment cost.

We reserve the right to cancel appointments if a client is more than 15 minutes late without prior notice.

5. Membership Billing and Cancellation

If you enroll in a membership program through Forever Rejuvenation Wellness, you agree to the following terms:

• Recurring Billing: Your membership includes recurring charges at the frequency and price agreed upon at enrollment
• Cancellation: Membership can be cancelled at any time with written notice of 7 days prior to your next billing date
• No Contracts: Forever Rejuvenation Wellness membership programs do not include long-term contracts. You may cancel at any time with proper notice
• Billing Disputes: Contact us immediately if you notice unauthorized or incorrect charges

6. Refunds and Credits

Wellness services and purchases are generally non-refundable. However, if you wish to dispute a charge or believe you have been overcharged, please contact us within 30 days of the charge. Membership cancellations will end recurring charges but do not entitle you to refunds for already-paid membership fees.

7. Limitation of Liability
